Download and complete the SR (Service Request) Form from the link at the bottom of this page (on the right).
Page 1 of the SR Form is used to identify the customer and set service preferences. Please include a phone number / email address in case we have questions and need to contact you.
Page 2 of the SR Form is used to identify the equipment being sent in. List the Model #, Serial #, and a brief description of the Problem or Error Code. (Note: iVIZION Heads and Transports are serviced as separate items. List each Head / Transport and their serial # separately).
When the SR Form is completed, please attach, and email it to us. Our Service Repair Coordinator will respond, contact you, and assign an SR number for your Service Request
When shipping your equipment to us, please use a durable cardboard box, and pack items carefully to avoid additional damage due to unforeseen bumping during transit. Don’t return any accessories.
Please write the SR # you were assigned on the outside of the shipping box, and please include a copy of the SR Form inside of the box with your equipment. An assigned SR is valid for 30 days.

The warranty period is based on when your shipment is received and inventoried here at our warehouse. If there are any shipping discrepancies, we will contact you.
